LEARNER STATUS - MONTHLY REPORT
NOTE:
Each Faculty Mentor is required to electronically submit this report to the department chair at the end of each month. The approved report will be sent to Accounting along with a request for payment.
The Learner Review is a summary for your information. The input of New Learner Contact information appears on the individual Learner pages.
Learner Name, Contact Information, Academic Advisor, Courses Completed:
This information appears in the upper-left side of the Learner Page.
Contact Date:
This is the date when the Mentor and Learner last communicated.
Type of Contact:
The variety of types of contact are reflected in the drop-down menu in the New Contact Information form found on the Learner Page. Course Participation requires Learners to make weekly academic contact with the mentor.
% Complete:
This may be a percentage or a fraction indicating the amount of required course work (number of assignments or some other measure of progress) that has been satisfactorily completed.
Comments Regarding Learners:
These may be notes on Learner progress, illness, travel, or other factors that impact course completion - or - notes on Learner habits or general capability; e.g., English proficiency.
Comments Regarding Courses:
Include ideas you may have for changing the course, or comments from Learner evaluations that are related to specific courses or our curriculum in general, especially things that you might want to share with other Mentors.
Grade:
The grade the Learner earned in the course will be recorded by the Registrar after the grade is posted by the Faculty Mentor.
Faculty Fee:
The amount the faculty Mentor is to be paid for mentoring or other authorized service to the University.
CA:
Denotes department Chair Approval of a Faculty Fee.
This report is used by:
- The Department Chair who approves faculty fees and verifies that each Learner is making satisfactory academic progress. The department chair also notes the number of Learners assigned to each mentor, the frequency of enrollment in various courses, etc.
- The Learner Affairs Advisor who tracks the academic progress of Learners and monitors Learner perceptions of the quality of administrative and academic services provided by the University.
- The Executive Vice President/ Provost who reviews the overall academic delivery system as it relates to number of faculty, average mentor assignments, Learner completion statistics, and comparisons between academic departments.

Received the first completed assignment. 9/26/00. The first assignment has been evaluated. The Learner earned 80 points (B-) in the assignment. It was poorly written with several grammatical errors. The Learner was reminded to proofread the papers before forwarding them for evaluation.
