EXHIBIT H: PROCTORED
EXAMS
If a faculty mentor chooses a proctored examination as one of the assessments for a course,
1. At the beginning of any course that requires a proctored exam, the mentor will ask the Learner to identify his or her proctor. Suitable proctors may be a local librarian, someone in the HR office of the Learner’s employer, or other disinterested party. The proctor may not be related or live in the same household as the Learner, or be an employee (subordinate) of the Learner.
2. The Learner will provide to the mentor the name and contact information of the proctor.
3. When the time comes to administer the proctored exam, the mentor will send the exam directly to the proctor along with the proctor form (attached to this procedure), and any conditions for the exam (maximum time, use of books or notes, etc.)
4. The proctor will administer the exam, and then return the completed exam papers to the mentor along with the completed proctor form.
The faculty mentor will include the proctor form along with the graded exam when this assignment is copied to the University for its records.
